Hormonal contraception and desire: what actually changes, in plain language

The pill's effect on libido is real for some people and negligible for others. The research on SHBG and free testosterone explains why the experience varies so much.

Ask ten people on combined hormonal contraception whether it changed their sex drive and you'll get answers all over the map — no change, a clear drop, occasionally an improvement once cycle-related dips or period-related pain disappeared. All of those answers can be true at once, because hormonal contraception doesn't affect desire through one simple lever. It affects a hormonal system with several moving parts, and where a given person lands depends on which parts move most for them.

The mechanism worth actually understanding: SHBG

Combined oral contraceptives (containing both oestrogen and a progestin) reliably increase the liver's production of sex hormone binding globulin, or SHBG — a protein that binds to sex hormones in the bloodstream, including testosterone. Testosterone, despite being culturally coded as a male hormone, plays a real role in desire for people of any sex; it's present in far smaller amounts than in men, but it isn't negligible to libido.

The relevant detail is that bound testosterone is not the same as available testosterone. SHBG binds it up, and only the unbound, "free" fraction is considered biologically active for things like desire. Research measuring hormone levels in people on combined contraceptives has found SHBG levels can rise substantially, and free testosterone can drop correspondingly — sometimes to a fraction of pre-pill levels. This is the clearest, most consistently replicated mechanism by which hormonal contraception can lower libido for some users.

Why "some" is doing a lot of work in that sentence

The size of the SHBG increase and the resulting drop in free testosterone varies meaningfully between different formulations, and varies between individuals on the same formulation, because baseline hormone levels and sensitivity to hormonal shifts differ from person to person. Some people experience no noticeable change in desire at all. Others notice a clear, unwelcome drop that doesn't resolve with time on the same pill. "The pill kills your libido" and "the pill doesn't affect libido" are both overstatements of research that actually says: there is a real physiological mechanism, it affects some people meaningfully and others barely at all, and there's no reliable way to predict in advance which group any individual will fall into.

The other side of the ledger

It's worth being fair to the full picture, because contraception's net effect on a person's sex life isn't only about testosterone. For people whose baseline cycle involved painful periods, unpredictable bleeding, premenstrual mood symptoms, or fear of pregnancy that made sex more stressful than pleasurable, hormonal contraception can remove enough friction that their experience of desire genuinely improves, even if free testosterone technically drops. Desire isn't purely hormonal — pain, anxiety and stress are all desire inhibitors in their own right, and any method that reduces those can help more than a small hormonal shift hurts.
